Zedmed Cloud request: Add a new user

This form is for Zedmed Cloud customers to request user access to Zedmed Cloud.  The request will go to the support team who will set up a 2FA cloud login and a Zedmed account. These requests are processed with a 1 business-day SLA.

Important: a new user request must be submitted by a practice manager.

For practitioners : we recommend adding the Doctor Details before requesting a Zedmed account for a doctor.
